Prohealth Care's services include:
Accommodation and Tenancy Assistance
Daily Personal Activities
Assisting with and/or supervising personal tasks of daily life to enable the client to live as autonomously as possible. These individual supports can be provided in a range of environments, including but not limited to, the client’s own home.
Assistance with daily tasks in group or shared living environment
Assisting and supervision of daily tasks in a shared living environment (temporary or permanent living arrangements) so that each client can live as autonomously as possible.
Assistance with travel and transport arrangements
A client is allocated transport funding if they cannot use public transport without substantial difficulty as a result of their disability. We will assist these clients with transportation to everyday commitments including to medical or social appointments, shopping, work, school or university.
Community Nursing Care
This is the provision of specialist care for clients who have high care needs requiring a high level of skill, and for the training of support workers to respond to the client’s complex needs.
Development of daily living and life skills
Training and development activities undertaken by the client to increase their capacity to live autonomously, including supports that will enhance their ability to travel and use public transport independently.
Group and centre-based activities
Assisting clients to participate in group-based community, social and recreational activities.
Household tasks
Assistance with day to day household tasks that the client is unable to undertake. This includes activities such as meal preparation and delivery, house and yard maintenance, house cleaning and linen.
Participate in community, social and civic activities
Assisting clients to actively participate in activities outside of their home and helping to develop capacity to engage in these activities.
Supported Independent Living & Supported Disability Accommodation
Specialised housing designed for clients with extreme functional impairment or very high needs. SDA homes are constructed to be more accessible for clients. They enable better or safer delivery of other supports clients may need. At Prohealth Supported Living, we provide more than just accommodation – we offer a place to truly feel at home.
